Senior Reaps Benefits of Clara Cares

Many seniors suffer from multiple chronic illnesses like chronic obstructive
pulmonary disease (COPD), congestive heart failure, and/or diabetes. They
have several physicians, take a variety of medications, and find themselves
bouncing in and out of the hospital because their conditions have spun
out of control. Clara Cares, a medical house call program sponsored by
Clara Maass Medical Center, can help.
Dorothy Bailey, 79, a resident of Kearny, is one of 500 individuals who
gets a helping hand from the Clara Cares’ team of doctors, nurse
practitioners, pharmacists and physician assistants. They help coordinate
her care for her COPD, and ensure that she has regular health checks,
referrals to specialists, and follow-up care.
Clara Cares Nurse Practitioner Faith Fajarito says that the team goes to
great lengths to help keep seniors as healthy as possible. “Whether
it is follow-up visits, sorting out medication, providing transportation
to doctors’ visits or identifying adult day care services, we do
whatever it takes to help keep seniors as healthy as possible.”

Clara Maass Medical Center (CMMC) is an acute care RWJBarnabas Health facility,
offering high-quality medical treatment close to home for the residents
of Essex, Bergen, Hudson and Passaic counties. CMMC offers an array of
