Professional Practice Model

The professional practice model (PPM) of nursing is a schematic description
that clearly articulates how nursing is practiced in that respective health
system. The PPM demonstrates the manner in which nurses coordinate and
provide patient-centered care, achieve best patient outcomes, and the
manner in which nurses develop and perform professionally within their
organization.
The SBMC Nursing PPM was developed based on input and feedback from staff
nurses and nursing leaders at all levels. In May and June of 2016, a group
of 14 nurses came together for two 2 hour retreats and discussed how we
can use the guiding principles of our nursing theorist, Joanne Duffy and
incorporate into our PPM.
Our patients and families are at the center of this model and are surrounded
by four Puzzle Pieces that form the components that support our nursing
practice. The components were constructed from the common themes identified
by team at these 2 retreats. They are: Culturally Competent Interprofessional
Care, Shared Decision Making, Collaborative Partnerships, and Evidence
Based Practice and Innovation. Community and Global Health are at the
top outer perimeter of the triangle signifying our awareness of the changing
health care environment around us. Our organizational mission, ‘cchess’,
is listed at the base of the triangle as it foundation of every interdisciplinary
team member at SBMC. Our Nursing Values are listed in the outer blue circle
in the acronym of CARING.
